In order to

register a student for pre-K, kindergarten or after school programs, parents

are required to bring the following items to registration: a certified

birth certificate (must have raised seal), a property deed, original signed

lease agreement or an affidavit of residency. An affidavit can be obtained

ext. 122.

Parents must

also bring a current gas or electric bill displaying the parent's name

and address of service, and a photo ID of the parent or guardian registering

the student.

When a child

is accepted into the program, parents will be required to provide an immunization

record on the Georgia State form 3231 and eye, ear and dental record on

the Georgia State form 3300. For additional information and appointments

for immunizations, eye, ear and dental records, please contact the Fayette
