To nominate
an individual, you should:
List the name and address
of the candidate.
List each specific
accomplishment.
Tell how the accomplishment
was unusual and tell how much time was spent on it.
Explain if other people
were inspired or motivated by this person's actions.
Tell what the results of
the accomplishments were.
Write out
your responses and mail to:
